چکیده

Quantification of bone marrow plasma cells in multiple myeloma patients using conventional morphology is of limited prognostic value, while the merit of multiparameter flow cytometry immunophenotyping is still considered unproven. Here we compare the bone marrow plasma cell counts obtained by morphology and multiparameter flow cytometry and explore the potential prognostic impact of both techniques in 765 newly diagnosed, uniformly treated multiple myeloma patients. Although multiparameter flow cytometry generally yields lower plasma cell counts (median percentage of 11% vs. 40%, respectively; p<0.001), there is a significant positive correlation between the two techniques (R =0.46, p<0.001). Regarding prognosis, multivariate analysis selected the bone marrow plasma cell counts obtained by multiparameter flow cytometry as an independent prognostic factor for overall survival (p=0.007), supporting the incorporation of multiparameter flow cytometry immunophenotyping into the routine diagnostic evaluation of multiple myeloma patients and validating the clinical utility of bone marrow plasma cell counting by multiparameter flow cytometry approaches. (clinicaltrials.gov identifier: NCT00560053).

Study of Multiple Myeloma Occurrence in Patients With 5%-10% Bone Marrow Plasma Cells: A Case Series Regarding Revised IMWG Diagnostic Criteria

Introduction: Utilizing lower limit of bone marrow plasma cells (BMPCs) is the main existing criterion for diagnosis of multiple myeloma (MM). According to the revised international myeloma working group (IMWG) diagnostic criteria, the value of 10% is agreed among experts as the cutoff level for diagnosis. Symptomatic patients with BMPC above this value are identified as definite cases of MM. H...

The sensitivity of CD138 immunostaining of bone marrow trephine specimens for quantifying marrow involvement in MGUS and myeloma, including samples with a low percentage of plasma cells.

Accurate quantification of plasma cells in bone marrow samples is essential for the diagnosis, classification and prognosis of plasma-cell dyscrasias. Published comparisons between aspirate/trephine morphology, flow cytometry and immunohistochemistry are lacking. Bone marrow plasma cells from 100 patients with plasma cell myeloma or monoclonal gammopathy of undetermined significance were quanti...

Comparison of Minimal Residual Disease Detection by Multiparameter Flow Cytometry, ASO-qPCR, Droplet Digital PCR, and Deep Sequencing in Patients with Multiple Myeloma Who Underwent Autologous Stem Cell Transplantation

Multiple myeloma (MM) is a hematological malignancy with a poor prognosis, characterized by clonal proliferation of plasma cells in the bone marrow (BM). Relapse due to undetected minimal residual disease (MRD) is the leading cause of death among patients with MM. This review summarizes the methods and prognostic value of MRD assessment in BM and autografts from MM patients who underwent autolo...

Solitary bone plasmacytomas need to flow.

In this issue of Blood, 2 studies, one by Paiva et al (for the Spanish Myeloma Study group) and one by Hill et al (for the Leeds group [UK]), showed independently that flow cytometry of the bone marrow could identify patients with solitary plasmacytomas (SPs) at high risk for progression to active multiple myeloma (MM). In the first study, 71% of patients with solitary bone plasmacytoma (SBP) w...
